Answer to #2 ------------ You would treat the geopotential height tendency calculation in a fashion consistent with how you would calculate explicitly the GRVT if you were starting with GRV at two different times. Since it is eulerian, the final minus the initial value, divided by the change in time [in proper units] at a fixed earth-relative position.
